He was predeceased by: his parents, Ernest Ballard and Aletha Ballard (King); his son John Kevin Ballard; and his siblings, Rex Ballard and Thereesa Bogard.
He is survived by: his wife Karen Ballard (Walters); his children, Marla Farris (Shawn) of Dugger, IN and Billy Ballard (Janet) of Dugger, IN; his daughter-in-law Julie Ballard; his brother James Ballard (Peg) of Minnesota; and his grandchildren, Kellsey Ballard, Abigaile Reynolds (John), Misty Shrader (David), Autumn Maynard, Paige Price (Boaz), Kevin Ballard, Andrew Farris (Shelbi), Robert Maynard, A. J. Swaby, Grant J. Cox and Ayvin Wittgren. He is also survived by 16 great grandchildren and several nieces and nephews.
