He is survived by: his wife Edith Slack (Armstrong); his children, Terry Shane Slack (Mary Ann) of Greendale, IN and Lisa Mae Slayback of Dillsboro, IN; his grandchildren, Brendan Slack (Makenzie) of Hebron, KY and Corben Slack (Krystal Morris) of Covington, KY; his great grandchildren, Ezrah Slack and Luca Slack; and his brother Rick Slack (Donna) of Sweet Water, TN..
