Cook (Burkhart) of Decatur, Indiana; his wife Becky Cook of Decatur, Indiana; his daughters, Mandy Heiser (Tony) of Decatur, Indiana and Katie Adams (Joe) of Fort Wayne, Indiana; his grandchildren, Kevin Cook, Gabby Heiser and Matthew Heiser; his stepgrandchildren, Cody Adams, Tate Adams and JT Adams; his sister Darlene Noel (Russell) of Indianapolis, Indiana; his siblings-in-law, Sharon Rich of Decatur, Indiana, Bev Butler (Jim) of Decatur, Indiana and Kenny Straub (Marlene) of Monroe, Indiana; and his sisters, Vicki Steffen and Susy Brooks. He is also survived by many nieces and nephews.
