He was predeceased by: his parents, Harvey D. Shook and Bernice L. Shook (Weyer); and his siblings, Russel H. Shook, Lulu Glee Ayers, Katherine A. Hoverman and Rose Elaine Lichtenberger.
He is survived by: his wife Nancy M. Shook of Decatur, Indiana; his daughters, Lucille F. Burcham (Richard) of Logan, Geraldine L. Columber (Kerry) of Canton, Cynthia A. Chrisman (Troy) of Ray and Patricia L. Braun (Brandon) of Monroe, Indiana; his sisters, Jacqueline Hunter of Ohio City and Mary Alice Sanders (Charles) of Princeton, Indiana; and his grandchildren, Courtney Hazelip (Matthew), Christine Burcham, Catherine Stimmel (Seth), Kirby Columber (Rachel), Jennifer Sterner (Dan), Holly Columber, Andrew Columber, Roxanne Beavers, Cheyenne Tackett (Justin) and Laci Braun. He is also survived by nine great-grandchildren; and four step-great-grandchildren.
Preferred memorials are to Laurel Missions in Kentucky or Operation Christmas Child.
