He was predeceased by: his parents, Walter Mosak and Leanelda Mosak; and his nephew Brandon Mosak. He is survived by: his wife Brenda Mosak; his son Eric Mosak (Malissa "Annie"); his grandchildren, Matthew Mosak (Jessica) and Riley Mosak; his brother Mark Mosak (Kathy); and his niece Michaela Mosak.

In lieu of flowers, donations may be given in Tom's name to the Protection of the Virgin Mary Orthodox Church.
