He was predeceased by: his parents, John Kozub and Helen Kozub; and his brother Ronald Kozub. He is survived by: his fiancée Dr. Elizabeth Przeniczny; his sister-in-law Sharon Kozub; his nieces, Jennifer Lewandowski (Jeremy) and Kimberly Kozub; and his great-nieces and great-nephews, Kyle, Addie and Avery Lewandowski.
In lieu of flowers, donations may be given in Tom's name to Shriners Hospitals for Children.
