He is survived by: his wife Saundra Slack (Jacobson); his children, Tracy Thegze (Tom), Debbie Bachmann (John) and Chris Slack; his grandchildren, Samantha Bass (Steve), Nick Ferguson (Katie), Lauren Emmert (Phil) and Ben Slack; his great grandchildren, Henley, Lucy, Mason and Cru; and his brother David Slack (Kyra).
In lieu of flowers, donations may be made to St. Jude Children's Hospital.
