He was predeceased by: his brothers, Lil and Big Tony Dovichi; his grandmother Doris Jewett; his brothers-in-law, Donnie and Matthew Berg; his grandmother Judy Berg; and his close friend Mikey Pasternak.
He is survived by: his wife Beth Cohen; his children, Savannah, Kailey, Sienna, Jadon, Adri, Cali, Maddison, Connor, Elaina and Jameson; his grandson Carter; his parents, Jan Allee (Jim) and Doug Cohen; his siblings, Michele Cohen, Bobby Davenport (Kayla), Brandon Davenport (Amy) and Teresa Bales (Derrick); and his sister-in-law Niki Berg. He is also survived by his aunts, uncles, nieces, nephews, cousins and many good friends.
In lieu of flowers, donations may be given in Jay's name to his family.
