He was predeceased by: his granddaughter Madison. He is survived by: his wife Ruthann Graczyk; his children, Brandi Bezvin (Zhenya), Greg Graczyk, Keith Smith (Martha) and Kristen Bernard (Matthew); his grandchildren, Jacob, Madelyn, Spencer, Gabrielle, Grayson and Brayden; his parents, Ronald Graczyk and Barbara Graczyk; his siblings, Richard Graczyk (Kathy) and Rosemary Miklusak (Larry); and his sister-in-law Jill Golden. He is also survived by many nieces, nephews, and cousins.
In lieu of flowers, donations may be given in David's name to Unity Hospice or It's a Pittie Rescue in Monee, IL.
