He was predeceased by: his parents, Verlin McIntosh and Marcella McIntosh (Shook). He is survived by: his wife Lisa McIntosh (Hale); and his daughters, Samantha Carry (Steven) of Scottsburg, Indiana, Sara Tincher (Rayn) of Scottsburg, Indiana and Elizabeth Meyer of Holton, Indiana. He is also survived by 8 grandchildren.
