He is survived by: his mother Rosealee Ginther (Johnson); his stepfather Herman Ginther; his sons, Mike Mosurak (Lorie Parrett) of Cromwell, Jack Mosurak Jr. of South Bend and Brian Mosurak of Three Rivers, Michigan; and his brother Jim Mosurak of Oregon.
