He was predeceased by: his parents, Vola I. Surber and Ester L. Russell Surber; his siblings, Linda Kay and Gary Duane; his parents-in-law, Kenneth and Frances Zachary; and his son Jeffrey Allen Surber.
He is survived by: his wife Janice J. Zachary Surber (Jan); his sister-in-law Rozella Surber; his children, Gregory K. Surber (Michael Creasy), Michael Ray Surber (Lesa Ann Myers) and Janna Kay Surber; and his grandchildren, Ty Alan Gilstrap, Jeffrey Braden Surber and Jordan Kay Gilstrap.
In lieu of flowers the family requests donations to be made to the Animal Welfare League or charity of donor's choice.
