He was predeceased by: his brothers, Bobby, Harold and Chester. He is survived by: his children, Douglas Smalley, Wayne Smalley (Tami) and Laurie Cox (Mark); his grandchildren, Leah Smalley, Daniel Smalley, Abigail Smalley, Jared Smalley, Brittany Smalley, Breanna Smalley, Brooke Smalley, Nicole Sweet and Andrew Sweet; and his siblings, Richard Pollitt, Mary Price (Philip), Helen Tiffin and Max Pollitt.
Memorial donations may be given in Jack's name to Maranatha Baptist Church, Covington.
