He was predeceased by: his mother Carol "Sis" Day Moore. He is survived by: his parent Jerry "Doc"; his wife Teresa L. Shafer; his children, Jenifer Nicholle Whipker (Doug) of Columbus, Brian Thomas Moore (Maggie) of New Albany and Tyler D. Moore (Delaney) of Bennington, Nebraska; his grandchildren, Emilee, Mason, Max Moore and Miller Moore; and his siblings, Den Moore (Lisa) of Elizabethtown, Tim Moore (Kelly) of Taylorsville and Linda Lininger (Doug) of Columbus. He is also survived by several nieces and nephews.
In lieu of flowers, memorials may be made to the American Cancer Society or Elizabethtown Fire Department.
