She was predeceased by: her mother Virginia Firks; her husband Edwin J. Middaugh; and her sisters, Sandra Nerzig, Sharon Basinger and Donna Morgan.
She is survived by: her children, Darcy Hlavaty (Frank) of Auburn, AL and Scott Middaugh of Fort Wayne; and her grandchildren, Sean Middaugh, Dana Hlavaty, Kristy Coleman and Kathleen Hlavaty.
In lieu of flowers memorial contributions may be made to the Fort Wayne Children's Zoo.
