He was predeceased by: his parents, Larry G. Mullendore and Mollie Mullendore (Hathaway); his son Gary W. Mullendore; and his siblings, Juanita Myers, Harold and Ralph Mullendore.
He is survived by: his wife Jo Ann Mullendore of Colfax; his daughter Vicki Seigfried (Bob) of Mulberry; his sister Phyllis Craig of Kokomo; his grandchildren, Amy Hysong (Kevin) of Mulberry, Kerry Mullendore (Jenny) of Clarks Hill, Kris Mullendore (Jessica) of Colfax, Angie Messer of Romney, Stacey Brown (Danny) of Brookston and Brooke Willis of Colorado; and his great grandchildren, Brian Brown of Brookston, Jaycie Brown of Brookston, Kaleb Stewart of Brookston, Garrett Mullendore of Colfax, Kaden Mullendore of Clarks Hill, Kaylie Mullendore of Clarks Hill, Braden Mullendore of Clarks Hill, Elizabeth Kinslow of Kansas, Skyler Mullendore of Clarks Hill, Layne Horlacher of Clarks Hill, Forrest Messer, Ethan Messer of Romney, Lindsay Willis and Dillon Willis of Colorado.
