He was predeceased by: his grandfather Allen Hazelwood. He is survived by: his parents, Paul Lee Hazelwood (Liz Mitchell) of Morgantown and Amanda Elaine Hazelwood (Smith); his siblings, Emma Hazelwood of Churubusco and Lance Hazelwood of Shelbyville; and his grandparents, Dale Smith, Dana McKeen and Crystal Hazelwood.
