He is survived by: his parents, Juan Urieta, Sr. of Madison, Indiana and Tammy Lopez (Chastain) of Charlestown; his son Brayden Urieta; his siblings, Anna Chastain of New Albany, Adriana Lopez of New Albany, Lucia Lopez of Henryville, Indiana and Adam Urieta of Madison; and his nieces and nephews, Lanna Courtney, Lydia Courtney, Larry Courtney, Jr., Linkin Courtney and Abel Urieta.
