He was predeceased by: his parents, Donald L. Cook and Betty L. Dean Cook; and his brother-in-law Jerry D. "Jug" Hampton of Charlestown, IN.. He is survived by: his siblings, Marcia Hampton-McGuire (Benny) of Jeffersonville, IN and Donald L. Cook Jr. of Charlestown; his nieces, Amy Sad (Dan) of Charlestown, Jill Biven of Cumming, GA and Lauren Gulley (Roger) of Indianapolis; his great-nieces, Kendall, Isabella, Sophia, Kennedy and Alyse; and his great-nephew Connor.
Due to the COVID-19, a private graveside service will be held at Scottsburg Cemetery, Scottsburg, IN.
In lieu of flowers, the family has requested that donations be made in Gerald's memory to The LifeSpring Foundation of Indiana.
