He was predeceased by: his father Willard Franklin Douglas; his grandparents, Phyllis and Ernest Chalupa; and his great-grandparents, Ada and Dewey Cline.
He is survived by: his wife Susan Carroll-Douglas of Central, Indiana; his sons, Nathaniel Carroll-Douglas of Central, Indiana and Gabriel Carroll-Douglas of Central, Indiana; his mother Sandra Chalupa Douglas (Chalupa) of Lanesville, Indiana; his sisters, Lyn Douglas Powers (James) of Lanesville and Susan Ruth Douglas (Chad Coffey) of Lexington, Kentucky; his uncles, Stephen Chalupa of Grayson and David Chalupa of Ashland, Kentucky; his cousin Jeff Chalupa of Ashland, Kentucky; and his parents-in-law, Pam and Tom Carroll of Duluth, Minnesota.. He is also survived by several nieces and nephews.
