He was predeceased by: his parents, Patrick J. Forkin and Anna Gorenz Forkin; his sister Mary Louise Lorton Sommer; and his children, Christopher W. Forkin and Kathleen Forkin Wood.
He is survived by: his wife Barbara Ann Carver; his children, Sherry Grosse (Philip) of Ramsey, NJ, Colleen Kaelin (Larry) of Carmel, IN, Dr. Mary Pat Forkin (Robert Arthur) of Carmel, IN, Michele Spinell (Richard) of Barrington, IL and Shaun P. Forkin of Carmel, IN.; and his sister Kathleen Forkin Schwalb (Joe) of Dallas. He is also survived by nieces and nephews.
