He is survived by: his wife Mary Ann Cook; his children, Michael Cook (Brenda) of Carmel, IN, Steven Cook (Michelle) of Chapel Hill, NC and Sandra Cook of Bloomington, IN; his stepdaughter Laurie Benson (Tom) of Paoli, IN; his grandchildren, Michelle Diveley (Nigel), Samantha Hall (Drew), Sienna Cook and Eric Benson (Heather); and his great grandchildren, Ben, Sam, Molly, Davey, Ruger and Emerson.
