He was predeceased by: his wife Rebecca "Becky" Lundberg; and his father George Richard "Dick" Lundberg. He is survived by: his daughters, Emily and Anna Lundberg; his mother Joann Cotterman Lundberg; and his sister Amy Dozeman. He is also survived by 1 nephew and 7 nieces.

In lieu of flowers, memorial contributions may be made to the American Cancer Society in honor of Blake’s late wife Becky.
