He was predeceased by: his parents, Ronald Shook and Judy Shook of Garrett; his son Brandon Shook; his sister Lynn Boicourt; his brother-in-law John Haynes; and his friend Titan.
He is survived by: his wife Denise Haynes of Butler; his sons, Devin Shook (Allie) of Butler and Mike Shook (Krista) of Butler; his grandchildren, Adrian, Jase, Heithe (Pi) and Sophie; his brothers, Ronnie Shook (Margie) of Economy, Indiana., Tom Shook (Suzie) of Fremont, Ohio., Marc Shook of Garrett and Todd Boicourt (Emily); his mother-in-law Pauletta Black (Rick) of Clarkton, Missouri.; his sister-in-law Patricia Hurley of Richmond, Indiana.; and his close friends, Larry and Chris Ely of Montpelier, Ohio.. He is also survived by Several loving aunts, uncles, nieces, nephews and extended family.
