He was predeceased by: his parents, Franklin D. Cook and Thelma R. Cook (Stephens); and his brothers, Timothy Ray Cook, Jeffery L. Cook and Joseph Heigl.
He is survived by: his sons, Jeremy Dickhaus (Christy) of Athens and Jason Dickhaus (Anna) of Luka, Mississippi; his nephew Brian Cook (Krystal) of Indianapolis; his daughters, Brandy Cook of Brownstown and Julinda Stout (Nathan) of Seymour; his siblings, James V. Cook (Tina), Teresa Kuhn and Deb Wilson (Terry); his grandchildren, Julia O'Hair, Virginia Rieckers (Jayson), Brandon O'Hair, Preston Dickhaus, Samantha Dickhaus, Jameson Dickhaus, Isabella Dickhaus, Landon Dickhaus, Colton Dickhaus, Michael Stout, Katylen Stout, Beckley Dickhaus, Alexis Dickhaus, Davonte Cook, Jaustin Cook, Mariah Cook, Elijah Cook and Thomas Cox; and his great grandchildren, Madelyn Otte, Tucker Motsinger, Everly Rieckers, Waylon and Kyson.
