He was predeceased by: his parents, Rudolph Cook (Rudy) and Helen Cook. He is survived by: his wife Mellissa (Missi); his children, Jessica Cook, Jacie Schafer (Jacob), Jordan Little (Ethan) and Jared Cook (Alyson); his brother Rudy Lee Cook (Patty); his grandchildren, Emry Little, Elliot Little and Nash Cook; and his parents-in-law, Paul, Angie Brewer, Amy Brewer and Mindi Holmes (Jeff). He is also survived by four nephews; two nieces.
