She is survived by: her children, Michael R. Polk (Judy) of Boonville, IN, Karen Collins of Boonville, IN and Kathy Adams of Boonville, IN; and her grandchildren, Deniese Collins, Rachel Long (Ken), Lisa Polk, Shari Buhmeier, Jeff Polk (Carla), Tara Fisher (Bob), Shelli Adams and Jennifer Adams. She is also survived by 10 great-grandchildren; several nieces and nephews.
