He was predeceased by: his father Garry Clyde Frederick; his grandparents, Richard, Ruth Slaven, Mary and Robert Frederick; his father-in-law Leroy Kirkley; and his daughter Melody Slaven.
He is survived by: his wife Angela Slaven (Kirkley); his children, Destiny, Serenity and Harmony Slaven; his mother Pam Slaven-Wininger (Slaven); his siblings, Chris Frederick (Amanda), Tyler Frederick and Jamie Frederick; his mother-in-law Jeanie Kirkley; his sisters-in-law, Crystal Hodge (Donovan) and Sierra Kirkley; and his nieces and nephews, Jasper, Nevah, Kara and Noah Hodge.
Due to the Covid-19 Virus, a maximum of 50 people will be allowed in the funeral home at any one time and masks (face Coverings) will be required.
The family asks that all memorial contributions be made to Jenkins Funeral Home to help defray the cost of final expenses.
