He was predeceased by: his parents, Mike Kotlarsky and Ona Kotlarsky (Sherman); his former wife Marilyn Kotlarsky (Wiley); his daughters, Lynn Foote of Worthington and Lisa Kay Kotlarsky; and his brothers, Glenn, Robert and Mike Kotlarsky.
He is survived by: his wife Jo Ann Kotlarsky; his children, Denise Anderson (Scott) of Rising Fawn, GA., Beth Howard (Mike) of Owensville, IN. and Kelly Cazzell (Tom) of Boonville, IN.; and his grandchildren, Leo, Dwight Foote, Jason, Eric, Jared Fox, Logan Lee, Whitney Hunt, Kylie, Justin Howard, Avery and Dalton Cazzell. He is also survived by great-grandchildren.
