He is survived by: his wife Maxine Balkema of Berne, Indiana; his children, Julie Lewis (Marcus) of Germantown, Tennessee and Chris Balkema (Jill) of Channahon, Illinois; his grandchildren, Luke Balkema, Isaiah Lewis, Max Balkema, Corie Lewis and Kristin Lewis; and his siblings, Duane Balkema of Cocoa Beach, Alan Balkema (Monica Dignam) of Bloomington, Indiana, Margene Brown (Thomas) of Lafayette and Jane Clute (Dave) of Cicero, Indiana..
