He was predeceased by: his parents, Cecil Perkins and Rose Perkins (Cundiff); his brother-in-law Ed O'Connor; his niece Gina Walker (Qualkenbush); and his parents-in-law, Beatrice Sheetz (Mattingly) and Jesse Sheetz.

He is survived by: his wife Carol Perkins; his daughters, Kristi Proctor (Greg) and Kari Rayeski (Michael); his grandchildren, Ramsey Proctor, Ava Rayeski, Aubrey Rayeski and Spencer Proctor; his sisters, Kathleen Qualkenbush (late Gene), Lori Fray, Barbara Taylor (Ray), Janet Embree and Terri Dye (Darrel); and his siblings-in-law, Brenda Hall, Kathryn King, Paula Hilton (Michael), Linda Krueger (Michael), Bonnie O'Connor (Anthony Povinelli), Cheryl Shelley (Brian), Ken O'Connor and Robert. He is also survived by 10 nephews; and 11 nieces.
