She is survived by: her son Kenneth Polk of Indianapolis; and her grandchildren, Ashley Polk of Bedford, Shayna Thompson (Polk) of Orleans, Dennis Polk of Indianapolis and Kevin Polk of Atlanta. She is also survived by several nieces and nephews; as well as several great-grandchildren.
