He was predeceased by: his mother Tina L. Smith (Eberly); and his grandmother Barbara Sutton. He is survived by: his father Darrell Smith of Battle Ground; his children, Harley Perkins, Kelsey Perkins and Gavin Perkins; his siblings, Heather West, Tawnya Smith, John Smith (Juanita), Darrell Lee Smith and Joshua Perkins (Vickianna); his stepsiblings, Shane Perkins, Nicci Perkins and Justin Perkins; and his grandchildren, Aaron and Kyle. He is also survived by several nieces and nephews.
