He was predeceased by: his parents, Stephen F. Hayworth and Katherine Hayworth (Sims); his daughter Shannon Campbell; and his siblings, Barbara Hayworth, Donald 'Termite Hayworth, Nancy Hayworth, Jerry Hayworth and Rusty.
He is survived by: his wife Nancy L. Hayworth (Critser); his daughters, Sherry Bayless (Terry) of Bargersville, Cindy Rebennack (Dale) of Bargersville and Melissa Horton (Rick) of Greenfield; his brothers, Max Hayworth (Betty) of Morgantown and David Hayworth (Victoria) of Morgantown; his grandchildren, Kyle, Ryan, Seth, Keith Bayless, Amanda Fagan and Hannah Horton; and his great grandchildren, Oliver, Theo Fagan, Colton and Charlotte Bayless.
All attendees of the graveside service will be required to wear a mask at all times in an effort to promote a safe environment.
