"Suzy" Mayfield (Moffatt); his wife Suzanne R. "Suzy" Mayfield (Moffatt) of Bargersville, IN; his sons, Carson L. Mayfield and Ethan B. Mayfield; his parents, Gene Austin Mayfield of Winchester, IN and Velia Mayfield (Nunez) of Winchester, IN; his brother Lance Austin Mayfield; his sister-in-law Meghann Mayfield of Franklin, IN; his grandparents, Santiago Nunez of Winchester, IN and Lois Mayfield (Moore) of Winchester, IN; and his father-in-law Larry Moffatt of Columbus, IN. He is also survived by several uncles; aunts; and cousins.
