He is survived by: his mother Carol Jean Cook (Angerer); his sons, Austin William Cook and Brandon Michael Cook; his sisters, Patricia Marie Adams (Larry) and Julia Jean Nolte (Tony); his nieces, Erica Meagan Arnett and Jayla Ann Arnett; his nephew Luke Lawrence Adams; and his aunts and uncles, Bill, Judy Angerer, Ray and Jeanne Downey.

In lieu of flowers, memorial contributions may be made directly to the family.
