He was predeceased by: his parents, Fred and Carrie; and his sister Fay Johnson. He is survived by: his wife Jacqueline Underwood; and his children, Pamela Blakley, Donald Glen Underwood (Annmarie) and Cyndi Davis (Tony). He is also survived by husband, father, grandfather, and friend.
