He was predeceased by: his parents, Joseph Dolezal and Aurelia Dolezal (Wozniak); and his brother Dr. Donald Dolezal. He is survived by: his wife Lee Ann Dolezal; his son Wyatt Dolezal (Michelle Florentine); and his sisters, Cynthia Jones, Carolyn Shebel (Michael) and Lynn Donohue (Richard).
