He was predeceased by: his son Jeremy Cook; his father Lloyd Cook Jr.; his siblings, Rick Cook and Lucinda Biltz; his niece Jamie Cook West; and his nephew Andrew Biltz.
He is survived by: his sons, Justin Cook of New Haven and Jordan Cook (Mackenzie) of Fort Wayne; his grandchildren, Hadley Cook and Marshall Cook; his mother Naomi Cook of Avilla; his siblings, Randy Cook (Shon) of Garrett and Melinda Vincent of Auburn; and his sister-in-law Susan Cook of Auburn. He is also survived by several nieces and nephews.
