He was predeceased by: his wife Carolyn DeShon Slack; his parents, Harry Melick Slack and Emza Scudder Slack; and his stepmother Gladys Slack. He is survived by: his siblings, Naomi Coy, Barb Freels (Jack) and Herbert Slack; and his sons, Dennis Slack (Esther), Darrell Slack (Shelly) and Douglas Slack (Lori). He is also survived by eleven grandchildren and several great grandchildren.
