He is survived by: his wife Wendy Cook; his children, Amy Cook, Chuck Cook (Shelly), David Cook (Jaime) and Cameron Matthews; his siblings, Linda Welling (Richard) and Steven Cook (Cindy); his parents-in-law, Clifford "Jay" and Susan Crouse; his sister Michelle; his brother-in-law Larry Drechsler; and his nieces and nephews, Livi, Michael, Jake, Stephen and Britney. He is also survived by 7 grandchildren.
