He was predeceased by: his parents, George Otto and Maxine Otto (Brown); and his wife Thelma Otto. He is survived by: his sister Judy Laughlin; and his children, Phillip Apple Jr., Yolonda McConnell (Collin), Steven Otto (Sherri) and Kim Pitts (Shane). He is also survived by several nieces and nephews.
Due to Covid-19 and the governor's mandate, face coverings will be required.
