He is survived by: his wife Leslie Forrest (Huffman); his children, Jacob Forrest (Cherie) and Amy Ballman (Matt); his grandchildren, Gavin Ballman, Hayden Ballman, Branson Forrest, Pryce Forrest and Michaela Ballman; and his sister Janice Freiburger.
