He was predeceased by: his parents, Herbert Jacobowitz and Betty Jacobowitz. He is survived by: his wife Amy Jacobowitz; his children, Melissa Stout (Bryan) of Fort Wayne, Adam Jacobowitz (Erin) of Fort Wayne and Megan Coffman (Blake) of Hoagland; his grandchildren, Owen, Henry, Oliver, Lilly, Lincoln, Grant and Everett; and his siblings, Arthur Jacobowitz (Karen), Helene Castellon, William Castellon, Stan Bradtmueller, Paul Meyers (Rick), Robert Swygart and Glen Richardson.

Memorials may be made to the American Diabetes Association.
