He was predeceased by: his parents, Peter Ptasienski and Stella Ptasienski; and his siblings, Peter, Danial, Jules and Dorothy.

He is survived by: his children, Lisa Mertins (Keith), Amy Isom (Chris) and Alan Ptasienski; his grandchildren, Justin Voss (Katie), Michael Mertins (Kastyn), Makenzie Mann, Hannah Mertins (Austin), Briana Mann (Henry), Coleman Ptasienski, Makayla Weidener, Jasper, Katie and Matix Ptasienski; and his siblings, Joe Ptasienski (Joyce), Gene Ptasienski (Blanche), Annie Massimo, Ceil Stankewicz, Sally Melch (John), Betty Lewis, Stas Ptasienski (Sharon) and Lorraine Holland. He is also survived by six great grandchildren.

Per Ed's wishes, please dress in flannel shirts or Chicago Bears attire and address any memorials to the family.
