He was predeceased by: his parents, Stephen Chontofalsky and Helen Chontofalsky (Aitkins); his daughter Carolyne Alzaidy; and his sister Theresa Martin. He is survived by: his spouse Maxene Slack; and his brother Michael Chontofalsky.
