He is survived by: his children, Brandy Provaznik of Collinsville, Jacob Provaznik of Arnold and Ashley Provaznik of Alton; his siblings, Sandy Stamm (Chuck) of Wood River and Dan Provaznik (Joanne) of Maryville; and his nieces and nephews, Stephanie, Erica, April, Bob and Darrell.
