He was predeceased by: his brother Richard "Ricky" Melvin; his mother-in-law Julie Verda; and his close friend Robbie Black. He is survived by: his parents, Donald Pollack and Catherine Pollack of Joliet; his wife Julissa; his sister Angela Conard (Levi) of Hutchinson, KS; his nieces and nephews, Samantha, Shaylee Conard, Hudson Cibella, Freya, Luna and Theodore Boden "Bo" Brummel; his father-in-law Richard Verda (Tina); and his sister-in-law Janet Brummel (Randy) of La Salle. He is also survived by several aunts, uncles, cousins.
In lieu of flowers memorials may be made in his honor to The Code Green Campaign (http://www.codegreencampaign.org) since he was an advocate for mental health in first responders.
