She was predeceased by: her parents, Clarence Duffey and Alberta Duffey; and her husband Donald F. Slapak. She is survived by: her children, Gregory Slapak of Wheaton, Scott Slapak of Wheaton and Wendy Slapak-Halloran of Wisconsin. She is also survived by four grandchildren, and two great-grandchildren.
