He was predeceased by: his sister Elsie Jilek (Kopecky); and his wife Josephine Cycyota. He is survived by: his daughter Carol Smith (Ed) of Southport, NC; his grandsons, Michael Smith (Meredith) of Hillside, IL and Matthew Smith of Chicago; his brother Milton Kopecky (Nancy) of Fort Meyers, FL; and his brother-in-law Robert Jilek of Clarendon Hills, IL. He is also survived by numerous nieces and nephews.
